The application deadline for the Spring 2011 planting season is Thursday August 12th. You can download a form here and submit to the EPX office at 1940 S. 13th St. 19148 by 5pm on Thursday August 12th. The first two pages are all you need to submit. Keep in mind you can simply submit applications all year long, and they will simply be submitted at the next application deadline.
